How to file your claim
We support you and facilitate the claims process at each step, so your claim is processed as quickly as possible.
Find a healthcare provider
As a Group Benefits plan member you can use our healthcare providers search engine to find one of our preferred healthcare providers in your area. Make sure to have your certificate number ready.
For healthcare providers
If you’re a healthcare provider, you can check the coverage period for our international plan members and their dependents.
Online claims
As a group benefits plan member, you can submit a claim through our online benefits portal.
Mail claims
If you have a group health and/or dental claim that cannot be submitted online, simply print and complete the appropriate form below and mail it to the address on the form.
